We accept Visa, Master Card, Discover, AmX and Debit Cards. Cash, Local Checks that don't bounce are accepted. 

 

We help the uninsured. Discuss fees upfront in person.  It is sometimes hard to tell what procedures may be needed over the phone but we will always strive to help you afford "The Care You Derserve".  We also accept many types of health insurance. Medicare, Medicaid, Amerigroup, AvMed, Blue Shield, Molina, United Healthcare, Devoted, Simply, Humana PPO/POS, Aetna Medicare, Coventry Medicare and many employer groups. Please look for Dr George Jacobson in your insurance directory and contact us to see if we are still on your plan.  ty



 

Bills not paid after 30 days are subject to a $5 late for fee each additional billing cycle, so please pay your bill on time or call the office if you are having financial difficulty.  A bounced check is far worse than setting up a simple no penalty payment plan.  ty
