In order to register and treat you promptly, we request that our patients bring their current insurance information with them for each visit.
No coverage no problem. WHY PAY MORE!!!
$150 CONSULTATION & X-RAY (heel pain, foot pain, fracture, sprain).
INGROWN TOE NAIL Tx FROM $150. (Permanent correction available)
$50-90 ROUTINE NON-COVERED CARE (treat corns, callus, toenails)
INSURED PATIENTS:
We will bill your visit directly to your insurance provider if you supply an updated proof of insurance at each visit. Please note that the insurance policy is between you and your insurance provider; we are happy to file the claim for you, but the balance is your responsibility if your plan does not pay after 30 days. Any amount not covered by your insurance policy is due immediately after you receive your first invoice from us.
OUT-OF-POCKET HIGH DEDUCTIBLE AND UNINSURED PATIENTS:
WHY PAY MORE!!! THE EMERGENCY ROOM MAY BE 1OX THE COST.
FOR CHARGES OVER $20 we accept Visa, Master Card, Discover, Debit Cards. Cash and Local Checks that don't bounce are accepted.
Bills not paid after 30 days are subject to a $5 late fee for each additional billing cycle, so please pay your bill on time or call the office if you are having financial difficulty. A bounced check is far worse than setting up a simple no penalty payment plan. ty
MEDICARE:
We accept patients on the Medicare plan. As a courtesy, we will also bill your secondary insurance provider directly. You will be asked to sign an Advanced Beneficiary Notice for services not covered
under Medicare. You shall be accountable for covering these services.
